In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual (TM)s health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

The population of the world grew from 1 billion in 1800 to 7 billion in 2012, and over the past 50 years the mean temperature has risen faster than ever before. Both factors continue to rise, as well as health inequalities. Our environment is changing rapidly, with tremendous consequences for our health. We cannot exclude the possibility that the allied forces of poverty, social inequalities, climate change, industrial food and lack of governance will lead to a deterioration in the health of large sectors of the population. In low-income countries, while many of the traditional causes of death (infectious diseases) are still highly prevalent, other threats typical of affluent societies (obesity, diabetes, cardiovascular diseases) are increasing. Africa is not only affected by malaria, TB and HIV, but also by skyrocketing rates of cancer. The book argues that the current situation requires effective and coordinated multinational interventions guided by the principle of health as a common good. An entirely competition-driven economy cannot - by its very nature - address global challenges that require full international cooperation. A communal global leadership is called for. Before hospitals in the United States were fully integrated, there were Freedmen’s Hospital in Washington, DC; Homer G. Phillips Hospital in St. Louis, MO; and Provident Hospital in Chicago, IL. These general hospitals were dedicated to the in-patient care of Blacks and training of Black surgeons. Even earlier there was the Georgia Infirmary in Savannah, which was an in-patient infirmary for enslaved persons founded in 1832.

Wright, and Daniel Hale Williams, surgeons who led their fields and advanced the cause of civil rights. Before them were Alexander Thomas Augusta, the first Black man commissioned as a medical officer in the Civil War (1863); James McCune Smith, the first Black man to graduate from medical school (1837); and Onesimus, the enslaved Black man who showed the Reverend Cotton Mather the technique of inoculation for smallpox (circa 1710).

The improvement of southern, rural, and black health would become a top priority of the U.S. Public Health Service during the Roosevelt and Truman administrations. Karen Kruse Thomas details how NAACP lawsuits pushed southern states to equalize public services and facilities for blacks just as wartime shortages of health personnel and high rates of draft rejections generated broad support for health reform. Southern Democrats leveraged their power in Congress and used the war effort to call for federal aid to uplift the South. The language of regional uplift, Thomas contends, allowed southern liberals to aid blacks while remaining silent on race. Reformers embraced, at least initially, the notion of "deluxe Jim Crow"--support for health care that maintained segregation. Thomas argues that this strategy was, in certain respects, a success, building much-needed hospitals and training more black doctors. By the 1950s, deluxe Jim Crow policy had helped to weaken the legal basis for segregation. Thomas traces this transformation at the national level and in North Carolina, where "deluxe Jim Crow reached its fullest potential." This dual focus allows her to examine the shifting alliances--between blacks and liberal whites, southerners and northerners, activists and doctors--that drove policy. In the summer of 1964 medical professionals, mostly white and northern, organized the Medical Committee for Human Rights (MCHR) to provide care and support for civil rights activists organizing black voters in Mississippi. They left their lives and lucrative private practices to march beside and tend the wounds of demonstrators from Freedom Summer, the March on Selma, and the Chicago Democratic Convention of 1968. Galvanized and sometimes radicalized by their firsthand view of disenfranchised communities, the MCHR soon expanded its mission to encompass a range of causes from poverty to the war in Vietnam. They later took on the whole of the United States healthcare system. MCHR doctors soon realized fighting segregation would mean not just caring for white volunteers, but also exposing and correcting shocking inequalities in segregated health care. They pioneered community health plans and brought medical care to underserved or unserved areas. Though education was the most famous battleground for integration, the appalling injustice of segregated health care levelled equally devastating consequences. It is also no secret that these nineteenth-century gynecologists performed experimental caesarean sections, ovariotomies, and obstetric fistula repairs primarily on poor and powerless women. Medical Bondage breaks new ground by exploring how and why physicians denied these women their full humanity yet valued them as ?medical superbodies? highly suited for medical experimentation. In Medical Bondage, Cooper Owens examines a wide range of scientific literature and less formal communications in which gynecologists created and disseminated medical fictions about their patients, such as their belief that black enslaved women could withstand pain better than white ?ladies.? Lining's comments presaged ideas about blackness that would endure in medical discourses and beyond. In this fascinating medical history, Rana A. Hogarth examines the creation and circulation of medical ideas about blackness in the Atlantic World during the late eighteenth and early nineteenth centuries. She shows how white physicians deployed blackness as a medically significant marker of difference and used medical knowledge to improve plantation labor efficiency, safeguard colonial and civic interests, and enhance control over black bodies during the era of slavery. In less than four months, beginning with a staff of five, an obscure office buried deep within the federal bureaucracy transformed the nation's hospitals from our most racially and economically segregated institutions into our most integrated. These powerful private institutions, which had for a half century selectively served people on the basis of race and wealth, began equally caring for all on the basis of need. The book draws the reader into the struggles of the unsung heroes of the transformation, black medical leaders whose stubborn courage helped shape the larger civil rights movement. Immigration As a Social Determinant of Health by
Since 1965 the foreign-born population of the United States has swelled from 9.6 million or 5 percent of the population to 45 million or 14 percent in 2015. Today, about one-quarter of the U.S. population consists of immigrants or the children of immigrants. Given the sizable representation of immigrants in the U.S. population, their health is a major influence on the health of the population as a whole. On average, immigrants are healthier than native-born Americans. Yet, immigrants also are subject to the systematic marginalization and discrimination that often lead to the creation of health disparities. To explore the link between immigration and health disparities, the Roundtable on the Promotion of Health Equity held a workshop in Oakland, California, on November 28, 2017. This summary of that workshop highlights the presentations and discussions of the workshop.
Achieving Health Equity Via the Affordable Care Act by
Since its creation by the Institute of Medicine (IOM) in 2007, the Roundtable on the Promotion of Health Equity and the Elimination of Health Disparities has been fostering dialogue on racial and ethnic disparities in health and health care, examining the development of programs and strategies to reduce disparities, and encouraging the emergence of new leadership focused on health equity. For the past several years, a prominent topic of discussion within the roundtable has been the Patient Protection and Affordable Care Act (ACA). The ACA has multiple provisions specific to race, ethnicity, and language and other provisions with significant implications for racially and ethnically diverse populations. In April 2013, the roundtable held a workshop to address many issues surrounding the ACA, including expansion of coverage, delivery systems, and access points, service delivery and payment reform, public-private partnerships, and challenges to the safety net. This report summarizes the presentations and discussions from the workshop.
Flatlining : Race, Work, and Health Care in the New Economy by
What happens to black health care professionals in the new economy, where work is insecure and organizational resources are scarce? In Flatlining, Adia Harvey Wingfield exposes how hospitals, clinics, and other institutions participate in "racial outsourcing," relying heavily on black doctors, nurses, technicians, and physician assistants to do "equity work"--extra labor that makes organizations and their services more accessible to communities of color. Making Medicines Affordable by
Thanks to remarkable advances in modern health care attributable to science, engineering, and medicine, it is now possible to cure or manage illnesses that were long deemed untreatable. At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicines "and health care at large "more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual (TM)s insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugs "coupled with the broader trends in overall health care costs "is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. This report explores structural and policy factors influencing drug pricing, drug access programs, the emerging role of comparative effectiveness assessments in payment policies, changing finances of medical practice with regard to drug costs and reimbursement, and measures to prevent drug shortages and foster continued innovation in drug development. It makes recommendations for policy actions that could address drug price trends, improve patient access to affordable and effective treatments, and encourage innovations that address significant needs in health care.
The Private Sector As a Catalyst for Health Equity and a Vibrant Economy by
A critical component of the nation (TM)s economic vitality is ensuring that all Americans can contribute and prosper. Such contributions presuppose an intentional focus on achieving the highest levels of health possible, which requires that conditions in communities, schools workplaces, and other settings promote health and address the social determinants of health for all community members. Many organizations, in both the private and public sectors, have been establishing partnerships to further healthy workplaces and health equity in general. Many are taking the lead in producing economic growth that is inclusive and responsive to the nation (TM)s diverse needs and populations. Increasingly, private "public partnerships are emerging as ways of doing business. Additionally, a variety of new developments in health, health care, and community benefits obligations that are part of the Affordable Care Act have contributed to this interest in economic growth and health and in the creation of new partnerships. To examine past successes and future opportunities, the National Academies of Sciences, Engineering, and Medicine held a workshop in November 2015. The workshop focused on the potential of the private sector to produce a triple bottom line: economic opportunity (including workforce development) and growth, healthy work and community environments, and improved employee health. At the same time, participants looked beyond the private sector to public "private partnerships and to public-sector actions that combine opportunities for economic growth and good health for all. Advancing Health Equity for Native American Youth by
More than 2 million Americans below age 24 self-identify as being of American Indian or Alaska Native descent. Many of the serious behavioral, emotional, and physical health concerns facing young people today are especially prevalent with Native youth (e.g., depression, violence, and substance abuse). Adolescent Native Americans have death rates two to five times the rate of whites in the same age group because of higher levels of suicide and a variety of risky behaviors (e.g., drug and alcohol use, inconsistent school attendance). Violence, including intentional injuries, homicide, and suicide, accounts for three-quarters of deaths for Native American youth ages 12 to 20. Suicide is the second leading cause of death "and 2.5 times the national rate "for Native youth ages 15 to 24. Arrayed against these health problems are vital cultural strengths on which Native Americans can draw. At a workshop held in 2012, by the National Academies of Sciences, Engineering, and Medicine, presenters described many of these strengths, including community traditions and beliefs, social support networks, close-knit families, and individual resilience. In May 2014, the Academies held a follow-up workshop titled Advancing Health Equity for Native American Youth.
